Human Injury Involving Animals

Submit Report

All human injuries involving animals (even when no medical treatment is sought) are to be reported to the Institutional Animal Care and Use Committee (IACUC) and Environmental Health and Safety (EHS) using the Human Injury Involving Animals Report.

Both the IACUC Occupational Health Coordinator and EHS will receive automatic email notifications when the form is submitted and EHS will collaborate with the appropriate medical provider based on information provided in the report.

  1. Report the injury to your supervisor!
  2. Submit the Human Injury Involving Animals Report.
  3. Seek medical advice or treatment:
    • Faculty and Staff contact Employee Health and Wellness at 706-721-3418, Professional Building 1 - AD1301.
    • Students may contact Employee Health and Wellness or Student Health at 706-721-3448, Pavilion II - AF1040.
    • In case of emergency or after hours go to the Emergency Department.
