Workers' Compensation

which may provide medical and income benefits if you are injured on the job. Workers'
Compensation also provides benefits to your dependents if you die as the result of
a job-related illness or injury. Please review our procedures to follow if you are

Procedures to Follow if Injured
Accident/Injury That Required Medical Attention
- Employee shall report accident/injury to their supervisor within 24 hours.
- Employee shall complete Section A of the and submit to their supervisor.
- Employee shall complete and return to Human Resources.
- Supervisor shall complete Section B of the Employees Report of Accident/Injury Form and submit to Wes Norton at wnorton@augusta.edu in Human Resources, Benefits & Data Management.
- Supervisor reports accident/injury to DOAS workers' compensation toll free number
at 1-877-656-7475.
Accident/Injury That Did Not Medical Attention
- Employee shall report incident to their supervisor within 24 hours.
- Employee shall complete Section A of the and submit to their supervisor.
- Employee shall complete and return to Human Resources.
- Supervisor shall complete Section B of the Incident Notice Only Form and submit to Wes Norton at wnorton@augusta.edu in Human Resources, Benefits & Data Management.
