All requests must be approved by the data steward. Some requests may require additional information and/or approval from a data trustee. If you are an AU employee or student requesting to send a survey to all (or a large group of) students, faculty, and/or staff, you may request so via the Survey Request Form. The Designated Senior Official (DSO) must approve all requests. Some requests may require more information for approval. Please allow 10 working days for your request to be completed. Please note, if your survey is part of a research study requiring Institutional Review Board approval, the DSO approval letter must be part of the IRB application packet.
