Requests cannot be accepted via telephone.
Service Fee:
Verifications for residencies and fellowships are subject to the following fees, payable
by check. The fees cover the costs associated with the retrieval and processing of
verifications and references.
Fee Schedule
$0- Verifications of residents/fellows who are 0-2 years post program completion.
$50- Verifications of training dates, program specifics on training including procedures,
commentary on performance, RAM licensure, etc., on institution or agency provided
form. For graduates 2-10 years post completion.
$75- For graduates 10+ years post completion.
$35- Additional fee for case logs
$5鈥� Additional fee for any verification requiring notarization
Any graduate 2+ years post program completion wishing to obtain a copy of his or
her complete training record will need to remit $50. If you have any questions or
concerns about your verification request, please contact our office at 706-721-2076.
Verification services will be rendered after payment and a signed consent form has
been received by our office. Please allow 7-10 business days for verifications to
be completed.
